Médecins Sans Frontières (MSF) Doctors Without Borders) is an international, independent, medical humanitarian organisation that delivers emergency aid to people affected by armed conflict, epidemics, healthcare exclusion and natural or man-made disasters to populations in danger in more than 70 countries.
Ensure the proper functioning of the medical activities under his/her responsibility, defining and implementing MSF policies, protocols, and procedures, and guaranteeing the quality of healthcare in order to improve health conditions of the target population.
Accountabilities
As Senior Field doctor:
- Functional supervision of the medical doctors in the hospital or outpatient department in her or his speciality area providing clinical support. she/he supervises also other technical areas in the absence of corresponding specialists or Hospital Clinical Lead
- Supervision of the implementation of medical protocols, procedures and standards used in her/his specialty area, and other areas in the absence of corresponding specialists or Hospital Clinical Lead, ensuring that MSF clinical standards are adhered to at all time
- Liaises with technical referents where necessary to ensure delivery of patient care
- Lead ward rounds and organize clinical sessions
- Provide technical coordination and inputs under MAM/PMR lead for: o Referrals of patients following agreed referral protocols in the absence of Hospital Clinical Lead position o Acts as technical Focal Point in the Project for eHealth, telemedicine, radiology/POCUS, electronic diagnosis algorithms and Clinical Operational Research initiatives when appropriate.
- Works in close collaboration with Senior Nurse and/or Nurse Supervisor
Quality of care: these tasks are to be performed in the absence of a Hospital Clinical Lead and according to the field needs and activities ongoing. The Senior Field Doctor will be in the best position to perform those, but they could be shared with other team members upon request:
- Performs as Quality of Care Focal Point in the project and in conjunction with the MAM, NAM and Nurse Lead ensures the development and implementation of clinical quality improvement plans
- Oversees the implementation of the medical incident reporting policy and follow up of all medical incidents
- Collaborates and supports the ATB focal point in the rational use of antibiotics and taking this role of focal point when pertinent
- In close collaboration with medical and nursing managers, supports the implementation, monitoring and analysis of QoC indicators in the project
- Leads regular clinical review meetings to ensure quality assurance and continuous learning in place and/or (in Hospital set ups) is core member or chairperson of the in-hospital mortality committee
- In close collaboration with the laboratory manager and/or lab supervisor, supports the coordination between laboratory and imagery activities and clinical care provision.
Leadership and training:
- Support MAM/PMR in identifying training needs for all medical staff in core competences required for their area of practice in particular the ability to identify and escalate the management of acutely unwell and deteriorating patients.
- Provides on the job supportive training including spot checks and skills demonstration in her/his speciality area.
Monitoring and evaluation:
- Upon request from MAM/PMR, supports that patients’ records, and clinic registers are completed according to MSF requirements.
- Upon request from MAM/PMR, support in the use of established reporting formats for data collection, analysis and report on daily & weekly activities to MAM or Hospital Clinical Lead as required and follow notifiable disease protocols.
- Supports the MAM/PMR or Hospital Clinical Lead in doing the HMIS data analysis of different areas
Other:
- Support that all consultations with patients are in patient centred a dignified and appropriate manner in keeping with the Quality of Care standards and principles set out by MSF
- Act as focal point for patient and caregivers’ complaints and concerns and escalate as per MSF Patient Charter and Behaviour and Abuse escalation Policy. This task could be either done by MAM or Hospital Clinical Lead when available.
- In close collaboration with MAM/PMR, support that systems in place to maintain patient confidentiality
MSF Section/Context Specific Accountabilities
- Ensure that the medical team complies with admission and discharge criteria across the hospital by ensuring that the right patient is placed in the right place (ward) at the right time and proper coordination of patients’ discharge by analysing patients’ records to determine legitimacy of admission, treatment, and length of stay inside Boost hospital.
- Responsible for effective implementation of admission process, comprehensive discharge criteria and coordination with other services (HP/Mental Health/ and other non-acute services) for each ward.
- In close coordination with heads of departments, analyses, monitors and forecast trends in respect to admissions.
- Review pre-admission, admission and transfer protocols, guidelines, and policies on the management of emergency workload and the timely placement of patients in the most appropriate environment in the Hospital in accordance with MSF standards.
- Coordinate and guide the implementation of the Hospital’s admission and discharge policies in consultation and in collaboration with the medical care coordination team and hospital management, ensuring that the decision-making process is effective, and that remedial action is taken where necessary.
- Regular, timely reporting of bed and resource utilization to hospital management and circulating regularly reports to key clinical decision makers on resource utilization.
- Regularly review with ward supervisors and medical staff to find beds for patients who need them as part of unscheduled care.
- Oversee all scheduling, daily admissions, and bed management, to balance the access demands of the Emergency department and those of urgent/non-urgent elective work with available beds.
- Facilitate twice-daily 10-minute bed management meetings with the department supervisors. One in the morning and one in late afternoon near the end of the shift.
- Maintain a live bed plan of each ward, displaying when a bed is occupied, and when a bed is available for a new admission.
- Join the patient rounds on daily basis, reviewing that patients are admitted within the admission criteria, taking note of length of admission, and reviewing with the treating MD patient readiness for, or timeline to, discharge.
- Time after patient rounds spent largely in the ER monitoring for new admissions, communicating to the appropriate ward about new patient admissions so they can prepare the bed. As well as, following up that planned patient discharges in wards have been completed, updating the live bed plan as this is achieved.
- Accountable for implementing admission and discharge criteria, (the ER MD supervisor is responsible) through direct interaction with the clinician to ensure adherence to admission and discharge criteria.
- Accountable for facilitating the consultation process between the ER MD and the MD of the anticipated ward for reception of new admissions, whereby the receiving MD (eg. ITFC or paediatric ward MD) will attend the ER for a physical examination and formal acceptance of the patient for admission within the admission criteria.
- Patient file audits upon discharge, using the file audit tool, as part of quality monitoring and evaluation. This process will also act as a tool to identify bottlenecks that contribute to high bed occupancy rates, which have potential opportunities for intervention or change in practice for best patient care (eg. frequent changes in antibiotics or diagnosis that prolong the patient admission; lack of adherence to admission criteria).
- Review patients with prolonged length of stay (>72 hours of antibiotic therapy) with the treating MD to assess for and propose readiness for discharge.
- Contribute to the hospital management meetings, to provide updates on bed management progress, challenges, and key moments achieved per the rollout and implementation timeline.
- Collaborates with Nurse Team Leader and Clinical Team Leader in developing and implementing a discharge plan
- Collaborate with Nurse Team Leader, Clinical Team Leader, and Hospital Coordinator in developing an education campaign to increase bed management knowledge.
- Coordinates transition of patient to outpatient setting (eg. follow-up in specialized OPD, MoP OPD or ATFC).
- Work closely with the Head of departments to improve data collection for accurate BoR measurement in each respective department, as well as ALS
- Flexible to perform additional work-related tasks requested by the Line manager or Functional manager

Experience
Minimum 3 years’ experience as a medical doctor in medical-related jobs. Essential
